-CAC expiring within 30 days
-Initial CAC/Retiree/Dependent ID card issue
-Rank/Name update
-Replacement of lost CAC

Initial CAC issues will require two forms of ID with one being a photo ID (driver’s license, SSN card, US passport, etc.)

For dependent ID cards, sponsor will need to be present with their dependent.

Dependents under the age of 10 are not authorized to be issued an ID card unless specifically stated otherwise.

Dependents ages 21+ require verification letter from their school verifying that they are a full-time student as the cutoff age for dependent IDs is 21.
