ID Cards

IMPORTANT: Please book ONE separate appointment for EACH person receiving an ID card. This gives us enough time to process paperwork, take photos, and laminate each card properly. Walk-ins are NOT guaranteed.

-CAC expiring within 30 days
-Initial CAC/Retiree/Dependent/Contractor ID card issue
-Rank/Name update
-Replacement of lost CAC (requires MFR from supervisor)

CAC issues generally require two forms of ID with one being a photo ID (driver’s license, SSN card, US passport, etc.).

For dependent ID cards, sponsor will need to be present with their dependent.

Dependents under the age of 10 are not authorized to be issued an ID card unless specifically stated otherwise.

Dependents ages 21+ require verification letter from their school verifying that they are a full-time student as the cutoff age for dependent IDs is 21.

Official Passports / Visas

Apply for US Passport. Must have:

- Prefilled DS-82 or DS-11 (complete application via https://pptform.state.gov/ and print)
- Copy (front and back) of CAC and Driver's License
- Two 2x2 passport photos
- Copy of orders
- Original Birth Cert or passport

For children under age of 16, a DS-11 is required along with the original birth certificate or naturalization document.

NOTE: One appointment per applicant.

Retirements

Dependent Enrollments (NO ID CARDS)

Before you book: Did you know that you can enroll your dependents into DEERS without having to come in person? Submit all forms and documentation to your CSS for faster processing!

Required for spouse enrollment:
- Marriage certificate
- Two forms of identification with one being a photo ID (driver’s license, US passport, SSN card, etc.)

Required for all other dependents (children, etc.):
- Birth certificate
- SSN card, if applicable
